A Direct Support Professional (DSP) is responsible for providing ongoing support for the individuals served by Elite Disability Services of Nebraska (EDS). They are tasked with a variety of duties related to the overall care of individuals with developmental disabilities live in a group home, independent living, and or shared family living setting. The DSP will assist with daily living skills (i.e., personal hygiene, cooking, and cleaning), social and leisure skills, and community activities.

Shared Living is a community-based living option for individuals with an intellectual or developmental disability. A Shared Living Provider (SLP) supports a person who comes to live in their home with daily living skills. SLPs have basic home support and related responsibilities, which include providing/supporting nutritional meals, providing transportation to all appointments, to/from day services and recreational activities, providing/supporting access to community activities, cultural and religious services, if appropriate, and providing up to 24-hour supervision and supports as outlined in the individual’s ISP (Individual Support Plan).
The individual served pays monthly room and board to the Shared Living provider. The subcontractor is paid through EDS for support services covered by state funds allocated for residential services.
Currently, EDS is seeking Shared Living providers (SLPs) to work in the Douglas County, Cass County, and Sarpy County area.

These programs focus on community partnerships, socialization, going into the community with a purpose, music culinary, and arts. DSP’s are also entrusted with providing exemplary services to both the individual served and their guardian(s), if applicable, and daily documentation of all support provided.

Full Job Description:
The Day Program Supervisor (PS) works to fulfill the EDS mission by operationalizing EDS core values. The PS is responsible for providing ongoing oversight to individuals receiving services by Elite Disability Services of Nebraska (EDS). The Program Supervisor is tasked with a variety of duties related to the overall care of individuals with developmental disabilities who attend our Community program. The PS ensures high quality standards are met through leadership, training, coaching, and oversight and quality assurance of work completed by the DSP’s.
Duties and Responsibilities:
· Represent and Support EDS administrative decisions in a positive manner
· Focuses and motivates staff and individuals receiving services to enable them to achieve EDS’s Mission, and 5 pillars of services
· Ensure DSP’s adhered to EDS policy and procedures
· Follow all set budgetary guidelines
· Conduct month drills and quarterly site visits to evaluate service delivery
· Protect the confidentially and rights of all individuals receiving services and supports
· Oversee staff schedules to ensure overtime stays withing guidelines and staffing is sufficient
· Completing DSP performance reviews
· Complete or assign annual assessments 90 days prior to Annual ISP
· Monitoring of data collected daily in Therap (i.e., T-logs, GERs etc.)
· Attend all ISP annual and semi-annual meetings for individuals on assigned caseload
· Assists with the development and implementation of ISP to ensure that regulations are followed specific to right restrictions, least restrictive alternatives, approval by RRC, ISP approval.
· Identify and secure community volunteering opportunities
· Serve as a member of the EDS administrative team and attend all required administrative meetings
· Maintains all applicable records in the form and manner described in the EDS policy
· Fill in and substitute for DSP’s when short staffed
· Direct care as needed when employee’s call out and severe staffing shortages.
